Lens Rebates will come directly fromCiba. Purchases must be made between January
1, 2006 , within 90 days of eye exam and July 31, 2006. All rebate requests must
be postmarked by August 31, 2006. Offer is limited to one (1) rebate per person.
Original mail-in certificate, original sales receipt and copy of valid eye exam
receipt are required. Allow 6-8 weeks for delivery. Offer valid only in the USA
